We are seeking a compassionate and skilled Home Care RN to provide high-quality healthcare services to patients in their homes. This role requires a commitment to patient-centered care, ensuring that each individual receives the support they need to maintain their health and well-being.
Duties
- Deliver personalized nursing care to patients in their home environment.
- Administer medications and monitor patient responses to treatments.
- Provide health coaching and education to patients and their families regarding health management.
- Assist with case management, coordinating care plans with other healthcare professionals.
- Conduct assessments for patients requiring specialized care, including infant care and internal medicine needs.
- Maintain accurate patient records and documentation of care provided.
Experience
- Previous experience in home community care or similar nursing roles is preferred.
- Experience in medication administration and case management is essential.
- Knowledge of health coaching techniques to empower patients in managing their health effectively.
