Job Summary
We are seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) to join our dynamic healthcare team. Shifts Needed:
2 pm - 10 pm M-F

Responsibilities
- Perform comprehensive patient assessments, including vital signs, physical examinations, and physiological evaluations to determine health status and care needs.
- Administer medications and injections accurately, including IV infusions and medication management aligned with HIPAA standards and medical documentation protocols.
- Manage complex care procedures such as catheterization, tube feeding, ventilator management, and wound suturing while maintaining aseptic techniques.
- Utilize electronic health record (EHR) systems like Epic, Cerner, or Athenahealth for accurate documentation of patient care activities and medical records.
- Assist with diagnostic evaluations involving laboratory specimen collection/processing, medical imaging coordination, and interpreting diagnostic results.
- Provide specialized care for patients with conditions such as dementia (including Alzheimer's and memory care), infectious diseases, acute pain management, and post-acute or hospice care.
- Support triage activities by evaluating patient symptoms in urgent care or emergency room settings to prioritize treatment pathways efficiently.
- Collaborate with multidisciplinary teams on discharge planning, case management, and community-based home & community care initiatives.
- Ensure compliance with infection control standards and maintain sterile processing during procedures requiring aseptic technique.
- Educate patients and families on health coaching topics such as nutrition, medication administration, disease management (e.g., diabetes or cardiac conditions), and post-hospital recovery.
Experience
- Valid LVN license in good standing with the state licensing board.
- Proven experience working in diverse healthcare environments including hospitals (Level I Trauma Center or Level II Trauma Center preferred), nursing homes, assisted living facilities, or outpatient clinics.
- Critical care experience in ICU or emergency medicine settings is highly desirable.
- Familiarity with EMR/EHR systems such as Epic, Cerner, Athenahealth or eClinicalWorks is required.
- Hands-on experience with IV insertion/infusion therapy, phlebotomy, venipuncture, and medication administration is essential.
- Knowledge of ICD coding (ICD-9/ICD-10) for accurate documentation and billing processes.
- Experience working with individuals with disabilities or developmental delays is a plus.
- Strong understanding of anatomy, physiology, medical terminology, and diagnostic evaluation techniques.
- Ability to work effectively in fast-paced environments such as urgent care centers or hospital units including PICU or Neonatal ventilator units. Pay: $30.00 - $34.00 per hour
