Position Summary:
Located in our West Chester, PA campus, the Case Manager will play a pivotal role in guiding clients through Synergy’s Program & supporting goal planning and goal execution. Critical to this role will be relationship building, and a professional, humble approach. This role reports to the Case Management Manager and will work closely with Primary Therapists, Recovery Care Coordinators, and the Client Needs Specialist.
Job Responsibilities:
Case Management Sessions
Session Facilitation & Goal Setting
- Conduct 60-minute weekly
meetings with clients to cover the following topics: daily living and wellness, 12-step engagement, relationships, work/school, budgeting.
- Create strategic SMART goals with each client to support their growth in key aspects of their experience at Synergy in alignment with their clinical work.
Administrative Support
Documentation & Collaboration
- Update and maintain
relevant client information according to standard company practices
- Collaborate with each client’s direct care team to resolve problems quickly and effectively – appropriately consulting with and informing necessary stakeholders.
Team Meetings & Supervision
- Attend weekly team meetings including the all-team, Program and Clinical staff meeting, Case Management Supervision meeting, and Management meeting.
- Participate in weekly 1-on-1 meetings with the CM manager to review work quality and receive direction and support related to the responsibilities of the job.
Scheduling
- Schedule sessions strategically around clinical, program, and client obligations to support clients in having a routine case management experience.

Qualifications:
- Minimum of 1 year of experience in substance use treatment or related field (Preferred).
- Highly organized and detail-oriented, with the ability to manage multiple tasks and priorities effectively.
- Strong leadership and supervisory skills, with the ability to effectively engage with clients, staff, and external stakeholders.
- Proven ability to manage crises effectively and provide support.
