Responsibilities
- Assess patients’ physical conditions through detailed evaluations, including mobility, strength, range of motion, and functional abilities.
- Develop and implement effective treatment plans aligned with physicians’ orders and patient goals, utilizing evidence-based physiotherapy techniques.
- Provide hands-on therapy interventions such as therapeutic exercises, manual therapy, trigger point therapy, and modalities to promote recovery.
- Collaborate closely with doctors, nurses, occupational therapists, social workers, and other healthcare team members to coordinate patient care and discharge planning.
- Document all patient interactions accurately in electronic health records (EHR) systems while adhering to clinical documentation standards.
- Educate patients and their families about treatment routines, home exercises, safety precautions, and community resources to support ongoing recovery.
- Monitor patient progress regularly and adjust treatment strategies accordingly to optimize outcomes across inpatient and outpatient rehabilitation settings.
Qualifications
- Valid licensure or certification as a licensed Physical Therapist in the relevant jurisdiction.
- Proven experience working in acute care hospital environments with a strong understanding of inpatient care protocols.
- Demonstrated ability to develop comprehensive care plans for diverse populations including geriatrics, pediatrics, neurology, orthopedics, and outpatient rehabilitation clients.
- Knowledge of physiology, anatomy, medical terminology, and physiotherapy techniques essential for effective treatment delivery.
- Experience with discharge planning processes and collaborating with healthcare professionals to ensure seamless patient transitions.
- Proficiency in electronic health records (EHR) management systems such as EMR platforms used in hospital settings.
- Strong communication skills for engaging with patients of all ages and working within multidisciplinary teams dedicated to patient-centered care.
