What You’ll Do
- Coordinate holistic care: collaborate with physicians, families, and caregivers to craft individualized Care Plans that meet medical, emotional, and social needs.
- Ensure plan adherence: monitor daily services, review progress toward goals, and retrain staff when updates are needed.
- Lead client advocacy: accompany clients to physician appointments or hospital visits; relay updates to the healthcare team and family members.
- Recruit & train caregivers: assist HR in hiring, onboarding, and educating field staff on best practices and agency protocols.
- Schedule like a pro: build and adjust caregiver schedules for a caseload of ~30–40 clients, balancing continuity of care and employee preferences.
- Track outcomes: document assessments, incidents, and goal progress in our EMR/CRM; generate reports that drive quality improvement.
- Problem‑solve fast: resolve urgent issues with calm, confidence, and compassion—24/7 on‑call rotation shared with team [if applicable].

- Lead by example: launch new cases, introduce permanent caregivers to families, and model best‑practice care techniques.
- Provide hands‑on care: assist clients with personal hygiene, mobility, light exercise, meals, housekeeping, errands, and medication reminders.
- Ensure quality & safety: complete home‑safety assessments, conduct routine quality‑assurance visits, and document plan‑of‑care updates for the Care Manager.
- Train & onboard caregivers: deliver one‑on‑one coaching, skills refreshers, and ongoing competency checks.
- Serve on the on‑call rotation: triage caregiver call‑outs, fill urgent shifts, and resolve client concerns after hours.
- Problem‑solve in real time: address changes in client condition, escalate issues, and keep families informe

What You’ll Bring
- Certification / license: CMA, CNA, LPN, RN, or Social Worker credentials welcome (preferred but not required)
1–2 years’ experience as a Care Coordinator, Client Care Manager, Case Manager, or similar role—home‑care or healthcare setting highly valued

Strong assessment, care‑planning, and documentation skills; familiarity with HIPAA and state regulations

Proficiency with Microsoft Office and EMR/CRM scheduling software
