Duties
- Conduct thorough patient assessments, including vital signs, physiology evaluation, and medical history review to determine care needs.
- Develop individualized care plans that incorporate discharge planning, diagnostic evaluation, and post-acute care strategies.
- Collaborate with healthcare providers across hospital medicine, emergency medicine, ICU, PICU, Geriatrics, behavioral health, and outpatient clinics to coordinate patient care.
- Manage utilization review and utilization management processes to optimize hospital stay lengths and resource use while maintaining quality standards.
- Facilitate intake procedures, review medical records, and ensure compliance with HIPAA regulations to protect patient confidentiality.
- Perform case management activities including patient service coordination, discharge planning, and post-discharge follow-up for inpatient and outpatient populations.
- Assist in diagnostic evaluations and medical record documentation to support accurate clinical decision-making and billing processes.
Requirements
- Valid nursing license with current registration in the applicable state or jurisdiction.
- Proven experience in critical care environments
- Demonstrated knowledge of physiology, diagnostic evaluation procedures, and medical terminology essential for effective case management.
- Prior experience with discharge planning, utilization management, and managing complex patient cases in inpatient or outpatient environments.
- Familiarity with hospital experience including medical records management and intake processes.
- Ability to perform comprehensive patient assessments including vital signs and functional status evaluations.
- Strong understanding of HIPAA regulations to ensure confidentiality and secure handling of medical information.
- Excellent communication skills for patient service delivery and interdisciplinary collaboration.
- Certification in case management or relevant nursing specialties is preferred but not required; however, a valid nursing license is mandatory.
