Job Summary
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic team. Duties
- Perform comprehensive patient assessments by monitoring vital signs, conducting physical examinations, and evaluating patient conditions using physiology knowledge.
- Administer medications and injections if needed, following prescribed protocols while ensuring compliance with HIPAA regulations for patient privacy and medical documentation standards.
- Document all patient interactions thoroughly within Electronic Health Record (EHR) systems like Noteable, ensuring precise medical records and ICD coding accuracy (ICD-9/10).
- Provide specialized care for specific populations as needed.
- Support discharge planning, case management activities, and coordinate with multidisciplinary teams to ensure seamless patient transitions across care settings.
- Complete an array of additional administrative duties.
Experience
- Proven experience working as an LPN in diverse healthcare environments such as hospitals or home & community care settings.
- Critical care experience in ICU or emergency room environments is highly desirable.
- Familiarity with EMR/EHR systems such as Noteable for accurate medical documentation.
- Hands-on experience with IV insertion, phlebotomy, specimen collection/processing, and sterile processing techniques.
- Knowledge of medical terminology related infectious disease care, hospice services, and pain management protocols.
- Experience working with individuals with disabilities (physical and mental) or behavioral and developmental challenges.
- Ability to perform physical assessments using anatomy knowledge; skilled in medication administration including injections and botulinum toxin treatments when applicable.

Pay: $26.00 - $32.00 per hour
