Join Our Team as a Licensed Master Social Worker (LMSW)!
Cardinal Hospice is seeking a compassionate and experienced Licensed Master Social Worker (LMSW) with hospice experience to join our team in a Part-Time position. As an LMSW, you will play a vital role in providing emotional, psychosocial, and practical support to patients and families throughout their hospice journey.
We are looking for an LMSW who is compassionate, adaptable, and dedicated to delivering exceptional end-of-life care while working closely with our interdisciplinary hospice team.
Position Summary
The Licensed Master Social Worker (LMSW) provides comprehensive psychosocial services to hospice patients and their families in accordance with the hospice philosophy of care. The LMSW will assess patient and family needs, develop individualized care plans, provide counseling and emotional support, and collaborate with the interdisciplinary team to ensure holistic, patient-centered care.
Essential Responsibilities
- Conduct comprehensive psychosocial assessments for hospice patients and families.
- Develop and implement individualized care plans as a Licensed Master Social Worker (LMSW).
- Provide emotional support, grief counseling, and crisis intervention.
- Assist patients and families with advance care planning and end-of-life decision-making.
- Connect patients and families with community resources and support services.
- Collaborate with physicians, nurses, chaplains, and other interdisciplinary team members.
- Maintain accurate, timely, and compliant documentation.
- Participate in interdisciplinary team meetings as an LMSW.
- Support bereavement services and family education as needed.
- Advocate for patients and families while ensuring dignity, compassion, and quality of life.
Qualifications
- Current, active Licensed Master Social Worker (LMSW) license required.
- Hospice experience required.
- Minimum of one year of hospice or end-of-life care experience preferred.
- Strong understanding of grief, loss, psychosocial dynamics, and hospice philosophy.
- Excellent communication and interpersonal skills.
- Ability to work independently while collaborating effectively with the hospice team.
- Compassionate, patient-centered approach to care.
- Valid driver's license, reliable transportation, and current automobile insurance.
