Local Travel Requirement: Travel within the Dayton area for client visits, outreach activities, meetings, and community engagement required
Requirements
- Experience in home care, healthcare, customer service, staffing, or coordination preferred
- Management or leadership experience preferred
- EVV system knowledge (Alora, Sandata, or similar) preferred
- Strong communication and relationship-building skills
- Strong organizational skills with the ability to multitask and prioritize responsibilities
- Professional, calm, and empathetic under pressure
- Comfortable with electronic systems and documentation
- Reliable transportation required
- Ability to work on-site Monday–Friday
Responsibilities
Client & Family Support
- Build supportive relationships with clients and families
- Assist with new client onboarding and follow-up communication
- Respond to concerns professionally and with empathy
Caregiver Engagement
- Support caregivers with updates, questions, and office needs
- Help resolve caregiver and client concerns in a timely manner
- Promote a positive, team-oriented work environment
Case Manager & Community Relations
- Communicate professionally with PASSPORT case managers
- Provide timely documentation, updates, and follow-through
- Participate in community outreach, provider meetings, and local events
Office & Compliance Support
- Maintain accurate client and caregiver records
- Assist with EVV visit corrections, compliance follow-ups, and system updates
- Support phones, filing, and daily office operations

Job Description
Wise Home Care is seeking a relationship-driven Home Care Coordinator to support clients, caregivers, case managers, and community partners from our Dayton office.
This is not a coordinating-only role. The Home Care Coordinator plays an important role in strengthening Wise Home Care’s presence within the Dayton market through client support, caregiver engagement, community outreach, and operational support.
This position is ideal for someone who enjoys building relationships, solving problems, and creating positive experiences while supporting individuals and families in the communities we serve.
