Health Reps is a respected multi-specialty healthcare team dedicated to providing compassionate, high-quality, patient-centered care. We are expanding our Chronic Care Management (CCM) program to enhance care coordination and improve outcomes for patients with chronic conditions.
Position Summary
We are seeking a compassionate, detail-oriented Patient Enrollment Specialist to join our team. In this role, you will educate eligible patients about our Chronic Care Management (CCM) program, explain the benefits of participation, and guide them through the enrollment process. You will serve as the primary point of contact for new patients, ensuring a seamless onboarding experience while obtaining required documentation and consent. Working closely with the care team, you will coordinate enrollment activities and maintain accurate patient records to help connect patients with the resources and services they need to support their ongoing care.
Key Responsibilities
- Educate eligible patients about the CCM program and obtain consent for enrollment.
- Provide ongoing support, education, and follow-up to enroll patients.
- Accurately document all patient interactions in EMR/EHR systems.
- Communicate with care teams to optimize patient outcomes.
- Maintain professionalism and deliver excellent patient service at all times.
Qualifications
- Minimum 1 year of experience in healthcare, medical assisting, or patient services.
- Strong communication, interpersonal, and organizational skills.
- Proficiency with EMR/EHR systems and patient data management.
- Detail-oriented, dependable, and able to work in a fast-paced environment.
- Knowledge of CCM, RPM, or chronic care programs preferred.
- Bilingual (English/Spanish) a plus but not required.
