Team Wellness Center is seeking a Children’s Intake Therapist to complete behavioral health assessments and connect children and families with appropriate services and ongoing care.
Responsibilities
- Complete intake and bio-psychosocial assessments, crisis plans, and required documentation.
- Assess mental health, behavioral, and co-occurring substance use needs.
- Facilitate Person-Centered Planning and develop appropriate treatment recommendations.
- Provide crisis intervention and referrals as needed.
- Utilize DSM criteria to support clinical diagnoses.
- Coordinate members with appropriate clinical staff and ensure follow-up appointments are scheduled.
- Maintain timely and accurate documentation in the Electronic Medical Record (EMR).
- Maintain compliance with agency, credentialing, and training requirements.
Qualifications
- Master’s degree in Social Work, Counseling, Psychology, or related behavioral health field.
- Current or eligible Michigan licensure: LLMSW, LMSW, LPC, LLP, or LP.
- Experience working with children, adolescents, and families preferred.
- CAADC preferred.
- Strong assessment, crisis intervention, communication, and documentation skills.
- Valid driver’s license and reliable transportation.

Salary: $50,000 - $60,000
Fully Licensed Therapist (LMSW, LPC, LP)
Salary: $70,000 - 75,000
