Home Health Registered Nurse (RN) responsibilities include:

providing services in accordance with the plan of care
-
making the initial evaluation visit and regularly reevaluating the patient's nursing needs, as well as ensuring the plan of care needs are met and making any necessary revisions

monitoring and evaluating patients' progress and updating care plans accordingly
-
educating patients and their families on managing the conditions and promoting wellness
-
informing physician and other personnel of changes in the patient's condition and needs, as well as collaborating with interdisciplinary healthcare team members to coordinate the best possible care
-
participating in weekend clinical on-call rotation.
- participating in in-service programs, supervising and teaching other nursing personnel as necessary
Requirements for the Home Healthcare Registered Nurse position:

valid Registered Nurse (RN) license in the state of Illinois

minimum of one year of nursing experience, preferably in home health or a related setting

proficient in using electronic medical record systems, such as Axxess
-
previous experience in geriatrics and home health
-
ability to work independently and prioritize tasks effectively
-
ability to provide care with empathy, respect, and understanding
-
strong communication and interpersonal skills
Job Type: Per Diem
Pay: Varies. Pay per visit based on Home Health experience
