Job Overview
We are seeking a dedicated and compassionate Long Term Care Licensed Practical Nurse (LPN) to join our healthcare team. Duties
- Perform thorough patient assessments, including vital signs, physical examinations, and medication administration, ensuring accurate documentation within electronic health record (EHR) systems.
- Administer medications, including injections and IV infusions, following established protocols and safety procedures.
- Provide specialized care for residents with dementia, Alzheimer's disease, or other memory impairments, including memory care activities and behavioral health support.
- Assist with tube feeding, catheterization, wound care, and other complex clinical procedures in accordance with best practices and infection control policies.
- Collaborate with interdisciplinary teams to develop individualized care plans that address residents' unique needs and preferences.
- Monitor residents' conditions closely, triaging urgent issues and escalating concerns to physicians or advanced practice providers as necessary.
- Support discharge planning and coordinate with families and caregivers to ensure seamless transitions of care.
- Maintain compliance with long-term care regulations, HIPAA confidentiality standards, and safety guidelines at all times.
Requirements
- Valid LPN license in the state of employment with current certification in basic life support (BLS) or CPR.
- Prior experience working in long-term care facilities such as nursing homes or assisted living communities is highly preferred.
- Demonstrated proficiency with EMR/EHR systems like Epic, Cerner, or Athenahealth for accurate documentation.
- Experience managing patients with complex needs including acute pain management, infectious disease care, and geriatrics.
- Knowledge of ICD-9/10 coding for medical documentation and billing purposes.
- Strong understanding of long-term care regulations, medication administration protocols, and patient assessment techniques.
- Excellent communication skills to effectively collaborate with residents, families, caregivers, and healthcare team members.
- Ability to handle multiple priorities efficiently while maintaining a compassionate approach to resident care.
