What You'll Actually Be Doing:
Out in the Community (Marketing & Outreach)
- Build relationships with hospitals, rehab centers, assisted living communities, physicians' offices, senior centers & more
- Visit referral partners and educate them about our services
- Attend health fairs, networking events & community activities
- Lead presentations & educational workshops
- Help with social media, email campaigns & outreach efforts
- Track referral activity and community contacts
Back with Our Clients (Care Coordination Support)
- Join in-home assessments and client visits
- Help build and maintain personalized care plans
- Coordinate service start dates & client onboarding
- Complete home visit documentation
- Follow up with clients & families to make sure they're happy
- Help resolve client & caregiver concerns
- Support our Client Care Coordinator team day-to-day
