POSITION OVERVIEW:
Mandy’s Farm provides services in the Albuquerque area for individuals with developmental disabilities, supporting them to live the life of their choosing.
The primary responsibility of a Program Manager at Mandy’s Farm is to manage a caseload of individuals with disabilities and the team of Direct Support Professionals (DSPs) who support them. The Program Manager reports to a Program Coordinator and collaborates with the ISP Specialist to ensure ISP implementation and compliance.

- Full Time Position; Rotating Schedule
- On Call Responsibilities: 1 week every 12 weeks

- Applicants must have a minimum 2 years of experience working with individuals with disabilities.
- Must have a valid driver’s license, car insurance and reliable transportation
- Demonstrate effective communication skills with others
- Must be available to work some weekends and evenings
PRIMARY RESPONSIBILITIES
- Understand and implement Mandy’s Farm mission, vision & values;
- Understand DOH/DDSD requirements and ensure program compliance;
- Have knowledge and understanding of all ISPs and other individual specific documentation in order to provide person centered supports;
- Assist persons served in identifying personal and ISP goals;
- Advocate with persons served to be heard in development of their outcomes;
- Monitor outcome progress and assist Direct Support Professionals in implementation;
- Promote and encourage the independence of all persons served through choice-making, the development of personal relationships, and the pursuit of other community connections;
- Provide supervision and support to all Direct Support Professionals, ensuring compliance with all regulations & company policies;
- Complete DSP Performance Evaluations in a timely and thorough manner;
- Serve as primary liaison for case managers and interdisciplinary team members;
- Assist in arranging team visits;
- Arrange and monitor individual specific training requirements, work with Training Coordinator to ensure DOH/DDSD training compliance;
- Coordinate and monitor persons served DME and assistive technology needs;
- Address concerns/suggestions in a timely manner;
- Addresses formal grievance (FARM forms) and take to Program Director if needed;
- Obtain and monitor stock of food, beverage and supplies for the residence;
- Serve as role model for persons served, DSPs and team members, exemplifying Mandy’s Farm values.
- Complete 5310 paperwork in a timely and accurate manner;
- Maintain regular communication with Scheduling Coordinator and ISP Specialist, work collaboratively to ensure program success;
- Ensure compliance with budgeted hours of service and and staffing hours;
- Ensure accurate completion of timecards, monitoring for accurate timecodes, differentials and hours worked;
- Monitor staffing needs & communicate to Program Director immediately any concerns;
- Educate and train DSPs on Mandy’s Farm policies & procedures, DD waiver standards, and individual specific training;
- Conduct safety drills as required, submitting appropriate paperwork and QA documentation to track compliance;
- Ensure responsible petty cash management, reconciling petty cash logs monthly, submitting to appropriate persons and documenting in Therap’s petty cash system;
- Attend interdisciplinary teams as scheduled;
- Work with the Health Care Coordinator to complete pharmacy reviews & monthly medication logs. Follow up on any medication changes, errors, and documentation errors;
- Coordinate overnight stays away from the residence, ensuring appropriate medication transfer;
- Share on-call responsibilities with team members.
