Responsibilities
- Evaluate and treat patients in their homes
- Develop individualized treatment plans
- Improve mobility, strength, and overall function
- Complete documentation and reassessments timely
- Collaborate with the interdisciplinary care team
Requirements
- Current Physical Therapist license
- Current CPR certification
- Valid driver’s license and reliable transportation
- Home Health experience preferred
