As a Home Health Physical Therapist, you will:

Perform and document comprehensive patient evaluations and develop individualized treatment plans.

Assess patient conditions and determine physical therapy diagnoses prior to intervention.

Design and implement treatment programs to improve mobility, reduce pain, restore function, and prevent disability.

Monitor patient progress and modify treatment plans to maximize outcomes.

Deliver care in accordance with infection control and universal precaution standards.

Collaborate effectively with patients, families, physicians, and interdisciplinary care teams.

Ensure timely, accurate documentation and compliance with company standards.

Promote the mission, values, and patient-first philosophy of Choice Health At Home.

Graduate of an accredited Physical Therapy program

Current Physical Therapy license in applicable state

Minimum 1 year of Home Health experience preferred

Current CPR Certification

Valid Driver’s License

Current Auto Insurance
-
Experience with HomeCare HomeBase preferred

Ability to stand, walk, sit, bend, kneel, squat, crawl, and climb for extended periods
-
Ability to lift, carry, push, or pull up to 100 lbs.
