Summary:
As a Case Manager at Aspire Health Partners, your job purpose to provide overall assessment and needs of chronically mental ill adults. These functions include – but are not limited to:
Responsibilities:
- Provide screening, referral, emergency, crisis and case management services
- Participate in the clinical treatment team
- Document clinical records and additional program specific documents while maintaining the integrity of the client
- Understand and use the necessary technology to communicate effectively with staff
- On occasion act as the authorized client transporter for necessary treatment and services
- Most importantly, all functions must be completed while maintaining high standards of ethical and professional conduct while adhering to agency polices and procedures
Qualifications:
- Bachelor's degree in Social Work, Psychology, or Human Services related field
- Acceptable Motor Vehicle Registration driver’s license record in accordance with the underwriting guidelines set by the Aspire insurance company as described in the AHP Corporate Transportation Policy 3.9.1
