Big Bend Hospice is seeking an experienced Registered Nurse Case Manager to provide compassionate hospice care to patients and families throughout Gadsden and Liberty Counties. As a Hospice RN Case Manager, you'll help patients remain comfortable in the place they call home while guiding families through one of life's most important journeys.

As a Hospice RN Case Manager, you'll coordinate and oversee all aspects of care for your assigned patients while serving as the clinical leader of the interdisciplinary care team.
Responsibilities include:

Managing an assigned hospice caseload throughout Gadsden and Liberty Counties

Completing comprehensive nursing assessments and developing individualized plans of care

Coordinating care with physicians and the interdisciplinary team

Managing pain and symptoms to maximize comfort and quality of life

Educating patients and families about disease progression, medications, and available resources

Supervising Hospice Aides and LPNs assigned to your patients

Documenting patient care accurately in the electronic medical record

Building trusted relationships with patients, families, healthcare providers, and community partners
These responsibilities reflect the RN Case Manager role, including coordinating the interdisciplinary plan of care, managing hospice patients, supervising Hospice Aides and LPNs, completing assessments, providing education, and ensuring quality care.
Required

Current Florida Registered Nurse (RN) License

Minimum of two years of inpatient nursing experience or previous hospice, home health, or long-term care experience

Valid Florida driver's license

Reliable transportation and current auto insurance

Excellent assessment, communication, and organizational skills

Passion for providing compassionate end-of-life care
Preferred

Hospice or Home Health experience

Case management experience

CHPN Certification
