Job Summary
Shift: Friday, Saturday, Sunday, Monday 8:00am to 6:00pm
License Required: PA RN License
Mileage Reimbursment: 0.62 cents/mile
Counties Covered: York and Lancaster counties
Required experience: Hospice and HCHB experience required. Candidate will be required to complete admission, routine, PRN, and death visits.
Productivity benchmark 6 to 8 visits per day.
RN License, Valid Driver's License, Valid Car Insurance, Valid BLS CPR Required
Duties
- Assess, plan, implement, and evaluate nursing care while promoting health maintenance, disease prevention, and case management for individuals in community or facility settings.
- Coordinate with physicians and healthcare teams to manage medications, monitor health changes, maintain accurate documentation, and ensure timely follow-up care.
- Supervise nursing staff, oversee care plans, and ensure compliance with health, safety, infection control, and medication management policies.
- Train and support employees on health, safety, and person-specific care needs while coordinating appointments, hospital admissions/discharges, and ongoing patient care.
