JOB DESCRIPTION
Agape Coordinating & Consulting, Inc. is a Support Coordination agency that provides case management services to children and adults who have varying developmental disabilities.
We are looking for a full-time Case Manager to work with families in need of supports and services through the Medicaid Waiver Program.
Responsibilities of a Support Coordinator
- Manage full caseload of already established clientele
- 80% in-office/remote work (telephone calls, related paperwork)
- Each TCM has the ability to work from home incrementally, based upon performance reviews and proven efficiency
- 20% on-the-road work (visiting clients in their home/day program settings)
Requirements
- Bachelor’s degree - NON-NEGOTIABLE (in any area)
- 2 years of experience in any related field of disabilities, case management, or counseling
- Own a reliable form of transportation

Pay: $40,000.00 - $47,000.00 per year
