General Definition and Scope of Job:
Reporting to the Manager of Substance Use and Recovery, with a home site of North Quincy, the Care Coordinator and Screening Navigator for Infectious Disease Services will provide screening, facilitate care, treatments and outreach to populations that are at risk for developing infectious diseases, HIV infection, Sexually Transmitted Infections (STI’s), and viral hepatitis. The Care Coordinator and Screening Navigator for Infectious Disease Services will partner with providers and collaborate on screening, education, counseling, access to care, and adherence support. Additionally, they will work in tandem with Manet’s primary care, infectious disease, behavioral health, recovery specialists and community partners to ensure a continuum of response and services.
Core Responsibilities:
- Maintain accurate, timely, and compliant documentation and reporting systems.
- Support HIV, Hepatitis C, PrEP and PEP services by coordinating screening, eligibility, baseline and follow-up lab testing, and ongoing patient engagement in collaboration with clinical providers.
- Track and follow up on laboratory results to ensure timely communication, appropriate treatment, and continuity of care.
- Track and renew required paperwork for patients to ensure no gaps in medications or continuity of care.
- Manage and support ongoing prevention services, including re-screening, follow-up appointments, adherence and retention in care.
- Conduct coordinated and targeted outreach and engagement efforts to increase access to screening and treatment, and prevention services among priority populations in Quincy, Attleboro and Taunton.
- Support community partners and community-based organizations in the prioritization of this work via direct services.
- Assist clients in navigating barriers to care, including insurance coverage, technology, language, transportation, housing, food, medication access, and appointment coordination.
- Collaborate closely with providers and care teams to ensure seamless coordination of services and health outcomes.
- Coordinate and collaborate on Narcan education and support distribution.
- Monitor and report on program outcomes and required metrics in alignment with organizational and grant requirements.

- Complete all necessary paperwork in a timely and accurate fashion and manage time appropriately to allow for completion of paperwork.
- Establish and maintain a good rapport and harmonious relationships with staff and patients, clients and community partners.
- Execute duties to reflect reasonable safety standards. Universal/standard precautions must be utilized and training obtained in areas that constitute risk.
- Must have dependable/reliable transportation as position requires travel to and from client sites, Manet locations, and external trainings.
- Perform duties consistent with Manet Community Health Center’s policies and procedures.
- Perform other related work duties as needed or as required by supervisor or designee.
Minimum Skills, Experience and Educational Requirements:
- High School diploma, Bachelor’s degree preferred
- Minimum two years professional experience.
- Knowledge of infectious disease: HIV, STI and Hepatitis transmission required.
- Demonstrates a level of comfort working with diverse populations; people recently released from prison, people who use drugs, LGBTQIA+ individuals, people experiencing houselessness, and people who do sex work.
- Phlebotomy experience preferred, willingness to learn required.
- Self-directed, flexible, excellent teaching and interpersonal skills.
- Displays excellent judgment skills.
- Excellent communication skills
- Demonstrates ability to strictly adhere to confidentiality requirements.
- Demonstrates strong attention to detail with excellent record-keeping skills.
- Has reliable transportation.
- Bilingual candidates preferred.

- Includes walking and standing for long periods of time, sitting for short periods of time, hand dexterity, clear hearing and speaking ability. Must be able to lift a maximum of 1/3 of their body weight.
- Excellent organizational and interpersonal skills.
Required Certifications and Licenses:
Must have Phlebotomy Certificate
Working Conditions:
- Works in well-lighted environment with comfortable surroundings.
- Stressful at times due to competing demands, including attention to multiple patients and staff.
- Will comply with all applicable laws, and will cooperate with the Manet Community Health Center compliance program rules and regulations.
Required Availability:
May be required to work evenings, holidays or weekends, if department requires.
Pay: From $50,232.00 per year
