Basic Function
This position is a frontline public health worker who is a trusted member of and/or has particularly close understanding of the community served. Position aims to address efforts focused on medical, behavioral, and social determinants of health factors that improve the well-being of individuals, families, and communities. Specific focus toward providing resources to pregnant community members. This role will link community members to community resources and assist in finding members that the Care Guides or Care Coordinators from Medicaid Care Plans could support. The position will be out stationed in a community-based organization, provider practice, or other central community location.
Must reside within Champaign County
Responsibilities
- Serve as an on-site connection between managed care organizations (MCO), community-based organizations (CBO), social services, healthcare providers/systems, and individual high risk or high acuity MCO members to facilitate access to services and improve quality and cultural competence of service delivery.

Serve as an advocate and community connection point for community residents to facilitate access to services and community opportunities.

Work collaboratively with existing MCO Care Coordination and Care Management to facilitate member connection to needed resources in the community and assist MCO, CBO, and member in closing loops.

Assist MCOs in finding/reaching members in the community with identified needs.

Provide culturally appropriate health education and distribute targeted outreach materials promoting Medicaid Manage Care Plan services/value-added benefits.

Advocate for clients when interacting with healthcare systems/partner services.

Provide community outreach by attending and representing the organization at community meetings and events in settings such as nonprofit community-based organizations, faith-based ministries, public schools, and other community-based organizations while providing culturally appropriate information through use of outreach materials and other avenues as appropriate.

Engage community members/clients to participate in the assessment processes including but not limited to screening for Medicaid, filling out report of pregnancy (ROPs), filling out health risk assessment (HRAs), and encouraging enrollment into Medicaid.

Work collaboratively with plan Care Guides or Care Managers for specified members to assist in plan of care execution for those members at a local community level.

Assist in identifying creative solutions to patient barriers and needs that may require innovative thinking to locate and refer to the appropriate resource(s).

Work with members to develop healthcare goals and identify potential barriers to achieving healthcare goals.

Utilize motivational interviewing techniques and active listening to collect and retain member information and incorporating responses as they are presented to complete assessment -Identify when supplemental assessments are needed and conduct supplemental assessments.

Evaluate effectiveness of community events and outreach materials.

Compile reports on outreach activities as requested.

Education
Associate's degree in a related area and/or Community Health Worker certification preferred. If not already certified, must complete approved Community Health Worker training and certification upon hire.
Experience
Familiarity with technology (computer, phone). Experience working in health/ healthcare, basic understanding of community organizations/ social services resources, outreach, case management, or home health experience is preferred. Experience working with similar populations in related role(s), such as social work or health education preferred.
