Full-Time Nurse Practitioner - Internal Medicine for a well-established, community-based health center in Brooklyn.
This role is ideal for a provider who values connection, consistency, and delivering high-quality care in an outpatient setting. You’ll work alongside a collaborative team focused on improving access and outcomes for a culturally diverse patient population.
- Deliver primary and preventive care to adult patients
- Perform assessments, physical exams, and manage chronic conditions
- Diagnose and treat acute and ongoing medical concerns
- Collaborate with physicians and care teams to ensure continuity of care
- Educate patients on health maintenance and disease prevention
- Document accurately within the EHR
- Engage in team meetings and quality improvement efforts
- Nurse Practitioner degree from an accredited program
- Board Certified as a Nurse Practitioner
- Active New York State license
- Experience in primary care or community health preferred
- Strong clinical judgment and communication skills
- Bilingual Creole/English strongly preferred
