Duties
- Conduct thorough patient assessments, including vital signs, medical history review, and physiological evaluations to determine individualized care needs.
- Deliver skilled nursing interventions such as wound care, medication management, IV therapy, and infectious disease control within the home setting.
- Collaborate closely with healthcare professionals—including physicians, therapists, and case managers—to develop and adjust patient care plans aligned with best practices and health regulations.
- Manage discharge planning and coordinate post-acute care to ensure seamless transitions from hospital or facility to home.
- Utilize Electronic Health Records (EHR) systems for accurate documentation of medical records, treatment plans, and progress notes while maintaining HIPAA compliance.
- Educate patients and their families on health management strategies, medication adherence, infection prevention, and safety protocols.
- Participate in clinical case management activities such as diagnostic evaluation, patient service coordination, and utilization management to optimize resource use and patient outcomes.
Qualifications
- Valid RN license with current registration in the state of practice.
- Proven experience in critical care environments such as ICU, emergency medicine, or Level I Trauma Centers is highly preferred.
- Demonstrated expertise in hospital medicine, post-acute care, or home health nursing with a focus on patient assessment and clinical case management.
- Strong knowledge of medical terminology, physiology, health regulation policies, and infection control practices including HIPAA standards.
- Experience with EMR/EHR systems for documentation and electronic health records management.
- Excellent communication skills for client interaction and collaboration with healthcare professionals across inpatient and outpatient settings.
- Ability to manage multiple complex cases efficiently while maintaining high-quality patient service standards.

Pay: $45.00 - $55.00 per hour
