Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods and equipment such as Hoyer lifts.
- Monitor and document patient health status by observing vital signs, recognizing changes, and maintaining accurate medical records in Electronic Health Record (EHR) systems.
- Administer medications and perform catheter care following home healthcare regulations and medical terminology standards.
- Support clients with meal preparation, light housekeeping, laundry, and cleaning to ensure a safe and hygienic living environment.
- Provide specialized dementia, Alzheimer’s, memory care, hospice, or end-of-life care tailored to individual needs while maintaining patient confidentiality in compliance with HIPAA.
- Communicate effectively with clients and their families to ensure understanding of care plans and address concerns promptly.
- Follow safety protocols including first aid, CPR certification requirements, and proper patient transfer techniques to prevent injury during caregiving activities.
Requirements
- Prior experience working with geriatric patients or individuals with developmental disabilities is highly preferred.
- Knowledge of medical terminology, vital signs measurement, medication administration, and basic life support (BLS/CPR).
- Experience with EMR/EHR systems for documentation; familiarity with home health agency procedures is advantageous.
- Ability to perform heavy lifting and assist with mobility using appropriate patient transfer methods such as Hoyer lifts or other equipment.
- Strong communication skills coupled with a compassionate approach to caregiving; patience and adaptability are essential.
- Valid certification in first aid and CPR; ongoing training in hospice care, dementia care, or home & community care is a plus.
- Compliance with all home health care regulations; ability to maintain patient confidentiality under HIPAA standards.

Pay: $18.00 - $20.00 per hour
