Responsibilities
- Conduct comprehensive assessments of patients’ physical function, mobility, and pain levels within the home environment
- Develop, implement, and modify individualized care plans based on patient needs, clinical standards, and best practices in physiotherapy
- Deliver hands-on therapy techniques including trigger point therapy, orthopedics, neurology interventions, and geriatrics-focused treatments
- Educate patients and caregivers on exercises, safety strategies, and activity modifications to support ongoing recovery and prevent re-injury
- Coordinate discharge planning with healthcare teams to ensure seamless transition of care and appropriate follow-up services
- Maintain accurate clinical documentation adhering to healthcare standards and electronic health records (EHR) management protocols
- Ensure compliance with all safety guidelines, infection control procedures, and regulatory requirements during home visits
Experience
- Proven experience practicing in a physical therapy role within outpatient clinics, inpatient hospital settings, or home health environments
- Demonstrated expertise working with geriatric patients, pediatrics, orthopedics, neurology, and community-based care programs
- Familiarity with outpatient rehabilitation center procedures and inpatient hospital protocols is highly desirable
- Strong knowledge of physiology, anatomy, medical terminology, and clinical documentation standards necessary for effective patient care
- Experience managing care plans that include discharge planning and collaborating with multidisciplinary teams
- Proficiency with electronic health records (EHR) or electronic medical records (EMR) systems for documentation and communication purposes
- Prior work in outpatient clinic settings or inpatient hospital environments that involved acute care experience is advantageous
