Job Overview
We are seeking a highly motivated and detail-oriented MDS Coordinator to join our healthcare team. In this vital role, you will oversee the comprehensive assessment and documentation of patient care needs within skilled nursing, inpatient, outpatient, or long-term care settings. Your expertise will ensure accurate completion of Minimum Data Set (MDS) assessments, supporting compliance with regulatory standards such as NCQA and CMS requirements. You will collaborate closely with interdisciplinary teams to optimize patient outcomes, ensure proper reimbursement through accurate coding, and facilitate seamless care planning. This position offers an exciting opportunity to impact patient care quality while utilizing your extensive knowledge of clinical documentation, medical coding, and healthcare systems. Responsibilities
- Conduct thorough MDS assessments in accordance with federal and state regulations, ensuring timely completion and accuracy.
- Review medical records, clinical documentation, and physician notes to verify data integrity and compliance with HIPAA standards.
- Utilize EMR (Electronic Medical Records) systems such as Epic, Cerner, Athenahealth, eClinicalWorks, or other EHR platforms to document patient information effectively.
- Collaborate with interdisciplinary teams—including nursing, therapy, social work, and case management—to develop individualized care plans aligned with patient needs.
- Perform utilization review and management activities to support appropriate level of care decisions for inpatient and outpatient settings.

Pay: $75,000.00 - $95,000.00 per year
