Job Overview
Life Choices Center is a faith-based, nonprofit organization committed to serving individuals and families with compassion, dignity, and respect. We are seeking a Registered Dietitian Nutritionist (RDN) and/or Certified Dietitian Nutritionist (CDN) to provide nutrition counseling, nutrition education, and food prescription services through the New York State Medicaid Food Voucher Program and other nutrition initiatives.
Responsibilities
- Conduct nutrition assessments for eligible clients.
- Provide individualized nutrition counseling and nutrition care plans.
- Prescribe nutritious foods in accordance with NYS Medicaid Food Voucher Program guidelines.
- Educate clients on healthy eating, meal planning, and chronic disease prevention.
- Provide services through in-person and/or telehealth appointments.
- Maintain required client documentation and reporting.
- Collaborate with Life Choices Center staff, healthcare providers, and community partners.
- Participate in staff meetings, trainings, and quality improvement activities.
Qualifications
- Bachelor's or Master's degree in Nutrition, Dietetics, or related field.
- Credentialed as a Registered Dietitian Nutritionist (RDN) and/or Certified Dietitian Nutritionist (CDN).
- Licensed or eligible for licensure in New York State.
- Experience providing nutrition education and individualized nutrition counseling.
- Strong organizational, communication, and technology skills.
- Experience with trauma-informed care preferred.
- Experience working with underserved, food-insecure, prenatal, or postpartum populations is a plus.
Ability to maintain client confidentiality and HIPAA compliance in a remote work environment.
Pay: $29.00 - $31.00 per hour
