Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor and observe patients’ health status, including vital signs and overall condition
- Administer medications following medical documentation and home healthcare regulations
- Support clients with medical procedures such as catheter care and wound management when trained and authorized
- Maintain accurate medical records using Electronic Health Record (EHR) systems and document patient care activities clearly
- Provide companionship and emotional support to promote mental well-being, especially for clients with dementia or Alzheimer’s disease
- Assist with meal preparation, light housekeeping, laundry, and cleaning to ensure a safe and comfortable environment
- Use proper patient transfer methods and equipment such as Hoyer lifts or patient transfer techniques to ensure safety during mobility assistance
- Follow HIPAA guidelines to maintain patient confidentiality at all times
- Communicate effectively with clients, family members, and healthcare teams to coordinate care plans
Requirements
- Proven experience working as a Home Health Aide or in assisted living, nursing homes, or hospice care settings
- Knowledge of medical terminology, medication administration, and vital signs monitoring
- Experience working with individuals with developmental disabilities or geriatrics is highly preferred
- Certification in First Aid and Basic Life Support (CPR) is required; additional certifications such as Alzheimer's or dementia care are a plus
- Ability to perform heavy lifting safely when required; experience with Hoyer lifts or patient transfer devices is advantageous
- Familiarity with home health care assistance regulations and documentation standards such as HIPAA compliance
- Strong communication skills for effective client interaction and team collaboration
- Valid driver’s license and reliable transportation are often preferred for client visits in the community
