GLC is seeking an experienced RN Case Manager for an assignment in Atlanta, GA. Position Details:
- Schedule: 5x8 Days
- Contract Length: 13 Weeks
- Start Date: ASAP
Compensation:
- Travel Pay: $20/hour
- Overtime Rate: $40/hour
- Weekly Stipend: $1,275
- Total Weekly Pay: $2,075 Weekly Gross
Job Requirements:
- Active RN License in Georgia or Compact
- Minimum 3 years of Case Management experience
- BLS required
- Strong discharge planning and care coordination experience
- Knowledge of Medicare guidelines, IMM notices, and MOONs preferred
- Acute care hospital experience preferred
Key Responsibilities:
- Assess, evaluate, and drive discharge plans for assigned patients
- Coordinate services to ensure patients are placed at the appropriate level of care
- Initiate, evaluate, and re-evaluate treatment plans daily
- Monitor patient progress and address barriers to discharge
- Manage length of stay and facilitate timely transitions of care
- Ensure compliance with Medicare regulatory guidelines, including patient choice, IMM notices, and MOONs
- Collaborate with physicians, nurses, social workers, and interdisciplinary teams
- Maintain accurate documentation and case management records
- Advocate for patient needs while promoting quality outcomes and resource utilization
