Oncall hospice visits on weekends

Provide accurate and timely documentation

Assess and provide patient and family/caregiver education

Perform other duties as assigned by the registered nurse

Other visit as needed based on your availability including routine and PRN, visits, discharges, Deaths, CC, etc.

Demonstrated compassion and commitment to patient centered care
-
Active LVN license in the state of California
-
Valid California driver’s license and reliable transportation

Minimum 1 year of hospice experience; strong understanding of hospice philosophy and end of life care

Prior experience in nursing or related medical fields

Experience as an on call LVN runner preferred
