Job description:
Prime Recovery Therapy LLC is a growing mobile outpatient physical therapy practice providing high-quality care to patients in their homes across Dallas County and surrounding areas.
We are seeking a motivated and compassionate Physical Therapist (PT) to join our team and deliver one-on-one care in a home-based setting.
Responsibilities
- Perform initial evaluations and re-evaluations and establish individualized Plans of Care
- Provide skilled physical therapy interventions based on patient needs
- Perform therapeutic exercises, gait training, balance training, and functional mobility training
- Utilize manual therapy and other appropriate skilled techniques as indicated
- Educate patients and caregivers on home exercise programs and safety strategies
- Monitor patient progress and modify treatment plans as needed
- Supervise Physical Therapist Assistants (PTAs) and ensure appropriate delegation of care
- Document all visits accurately and timely in EMR (HelloNote or similar)
- Ensure compliance with Medicare Part B guidelines and company standards
- Communicate with referring physicians and healthcare team regarding patient progress
- Travel to patient homes within Dallas County
Requirements
- Licensed Physical Therapist (PT) in Texas
- Graduate of an accredited Physical Therapy program
- Minimum 1 year experience preferred (home health or outpatient a plus)
- Strong clinical reasoning and decision-making skills
- Excellent communication and patient care skills
- Reliable transportation and valid driver’s license
- Ability to work independently and manage schedule efficiently
