Responsibilities
- Perform comprehensive patient assessments, including vital signs, physical examinations, and detailed medical histories to inform care plans.
- Administer medications and injections accurately following physician orders, including IV infusions and other specialized treatments.
- Manage catheterization procedures, tube feeding, wound care, and sterile techniques to ensure optimal patient comfort and safety.
- Utilize electronic health record (EHR) systems such as Epic, Cerner, or Athenahealth to document medical records, treatment notes, and medication administration meticulously.
- Assist with diagnostic evaluations like specimen collection/processing and coordinate with physicians for imaging or laboratory tests.
- Provide specialized care for patients with complex needs such as dementia, Alzheimer's disease, memory care, or behavioral health conditions.
- Support emergency response efforts by managing airway issues, performing CPR, first aid, and critical care interventions in acute situations.
- Collaborate with interdisciplinary teams on discharge planning, case management, and long-term care strategies to ensure seamless patient transitions.
- Maintain strict adherence to HIPAA regulations and clinical confidentiality policies while safeguarding patient information at all times.
Experience
- Valid LVN license with current state certification required; additional certifications in CPR/BLS preferred.
- Proven experience working in diverse healthcare environments such as hospitals (including ICU or trauma centers), skilled nursing facilities, assisted living communities, or home & community care settings.
- Demonstrated proficiency with EMR/EHR systems like Epic, Cerner, or eClinicalWorks; experience with ICD coding (ICD-9/10) is highly desirable.
- Critical care experience including ICU or emergency room settings is advantageous; familiarity with Level I & II trauma centers enhances your candidacy.
- Hands-on expertise in managing complex patient populations such as geriatric patients, individuals with developmental disabilities or those requiring hospice or palliative care.
- Knowledge of infection control protocols and sterile processing techniques to prevent hospital-acquired infections.
- Strong communication skills for effective caregiver communication and client interaction; experience working with children or neonatal ventilator management is a plus.
