Job Summary
We are seeking a compassionate and detail‑oriented Care Manager to join our team in Salina, Kansas. In this role, you will coordinate patient care plans, assess healthcare needs, and ensure timely, high‑quality service delivery. You will support patients and families with dignity, fairness, and open communication while promoting positive health outcomes through well‑coordinated care .
Responsibilities
- Coordinate individualized plans of care, ensuring quality, timely service delivery, and medical necessity of patient admission
- Assess, plan, implement, monitor, and evaluate care options and services to meet patient and family needs
- Promote efficient, safe, and well‑orchestrated care across all service areas
- Maintain an open, approachable, and respectful manner that preserves patient dignity and self‑confidence
Required Qualifications
- Bachelor’s degree or higher
- 2+ years of experience in Healthcare
- 1+ years of experience in any of the following:
- Case Management
- EHR Coordination
- Care Management
- Valid BSW in the state of Kansas (KS)
