Position Overview
As a Physical Therapist (PT), you will evaluate, plan, and implement evidence-based physical therapy programs to improve residents’ mobility, strength, balance, and overall functional independence. You will play a key role in interdisciplinary collaboration, patient and caregiver education, and ensuring high-quality clinical outcomes.
Key Responsibilities
- Evaluate residents to assess mobility limitations, strength deficits, balance, and functional impairments
- Develop and implement individualized treatment plans focused on mobility, gait training, strength, and pain management
- Monitor patient progress and adjust treatment plans as needed
- Supervise and collaborate with Physical Therapy Assistants (PTAs)
- Recommend assistive devices and adaptive equipment to promote safety and independence
- Educate residents, families, and caregivers on treatment plans and mobility strategies
- Maintain accurate and timely documentation in compliance with state and federal regulations
- Participate in interdisciplinary team meetings and care planning
- Ensure compliance with all regulatory and facility policies
Qualifications
- Active Texas Physical Therapist (PT) license
- In good standing with all relevant licensing boards and regulatory agencies
- Experience in long-term care, skilled nursing, or geriatric rehabilitation preferred
- Familiarity with Net Health documentation system a plus
- Strong communication, leadership, and clinical decision-making skills
