PathWays is seeking an Adult Services Program Specialist to join our Adult Services team! In this role, you'll help ensure individuals receive quality services, achieve their personal goals, and remain connected to their communities while serving as an important link between individuals, families, support teams, and service providers.

What You'll Do:
-
Manage a caseload of individuals receiving Adult Services
-
Coordinate assessments, Individual Support Plan (ISP) development, reviews, and implementation
-
Monitor progress toward goals and outcomes and recommend programming adjustments as needed
-
Collaborate with families, providers, support coordinators, and interdisciplinary teams
-
Ensure services are delivered according to each individual's ISP
-
Support community inclusion, participation, and relationship building
-
Maintain accurate documentation and electronic health records
-
Complete required reviews, reports, and incident documentation
-
Promote person-centered practices, positive supports, and Everyday Lives principles

Ensure compliance with ODP, OLTL, DHS, OVR, and PathWays requirements
What You’ll Need:
Associate Degree (or 60 college credits) and 4 years of experience working directly with individuals with disabilities
OR
Bachelor's Degree and 2 years of experience working directly with individuals with disabilities
OR
Master's Degree and 1 year of experience working directly with individuals with disabilities
Additional Requirements:

Valid driver's license

Ability to drive agency vehicles as needed
- Ability to obtain and maintain required clearances and health screenings

CPR, First Aid, and Medication Administration certification (training provided)
