ONLY 90 beds!
The salary range is between $ 75,000-$78,000 per year plus full benefits
There are two Social Workers in this building and ONLY 90 beds!
We are seeking a compassionate, dynamic, and experienced Social Service Director to lead our department in providing exceptional psychosocial care at our upscale Skilled Nursing Facility (SNF). In this role, you will champion resident advocacy, oversee comprehensive social services, and ensure our facility maintains the highest standards of emotional and social well-being for our residents and their families.
Key Responsibilities
Resident Assessment & Care Planning: Conduct thorough psychosocial evaluations upon admission, identify residents' social and emotional needs, and develop individualized care plans in collaboration with an interdisciplinary team.
Discharge Planning: Coordinate seamless, person-centered discharge and transition plans, collaborating with home health agencies, equipment providers, and community resources to ensure continuity of care.
Family Support & Counseling: Serve as a primary liaison and empathetic resource for residents and families navigating life transitions, adjustment challenges, and long-term care planning.
Regulatory Compliance: Maintain complete, accurate documentation in accordance with state, federal, and facility guidelines, ensuring survey readiness and adherence to CMS regulations.
Interdisciplinary Collaboration: Partner closely with nursing, rehabilitation, therapeutic recreation, and administration departments to foster a holistic, resident-focused environment.
Grievance Resolution: Actively listen to and promptly address resident and family concerns, facilitating swift, compassionate resolutions that uphold the facility’s luxury service standards.
Qualifications
Education: Minimum Bachelor’s Degree in Social Work (BSW)
Licensure: Current state Social Work licensure (LSW, LMSW, or LCSW) as required by state regulations.
ACSW Registration
Experience: Minimum of 1 year of social work experience within a Skilled Nursing Facility, long-term care or sub acute environment
