What You’ll Do:
- Provide skilled nursing care to patients in their homes
- Conduct comprehensive assessments and ongoing evaluations
- Develop and revise individualized care plans in collaboration with the care team
- Educate patients and families on disease process, medications, and care strategies
- Administer treatments and medications in accordance with physician orders
- Serve as Case Manager for your assigned patients, coordinating services and resources
- Communicate effectively with the interdisciplinary team to ensure continuity of care
Schedule Expectations:
- Full-time | 5 days per week
- Shared on-call rotation
What You’ll Need:
- Active RN license in the State of New Hampshire
- Graduate of an accredited nursing program; BSN preferred
- 1–2 years of recent acute care or home health experience
- CPR certification
- Valid driver’s license and reliable, insured vehicle
- Strong clinical skills, excellent communication, and a heart for community-based care
