Duties
- Provide comprehensive patient assessments, vital signs monitoring, and physical examinations to inform care plans.
- Administer medications, injections, IV infusions, and perform catheter care following strict protocols and safety standards.
- Assist with medical procedures such as tube feeding, venipuncture, sterile processing, and wound suturing.
- Support patients with activities of daily living, including mobility assistance with Hoyer lifts and personal hygiene tasks.
- Document all patient interactions accurately using Electronic Health Record (EHR) systems like Epic, Cerner, or Athenahealth while maintaining strict HIPAA compliance.
- Collaborate with multidisciplinary teams for discharge planning, case management, and diagnostic evaluations to optimize patient outcomes.
Experience
- Valid RN or LPN licensure with current state registration; CNA certification preferred for entry-level roles.
- Demonstrated experience in diverse clinical settings including hospital medicine, long-term care facilities, skilled nursing facilities, or assisted living communities.
- Background working with geriatric patients, individuals with developmental disabilities, dementia or Alzheimer’s disease; experience in memory care is a plus.
- Knowledge of infection control protocols, hospice care practices, pain management techniques, and acute/urgent care procedures.
- Strong understanding of physiology, anatomy knowledge, medical terminology, and diagnostic evaluation processes necessary for comprehensive patient assessment.

Pay: $16.00 - $36.00 per hour
