Assignment Details:
- Assignment Type: In-Home and Group Home Visits
- Location: Carbondale, IL
- Shift: 7am - 8pm (1h lunch break)
- Schedule: 3 Days per Week (Flexible), EOW
- Start Date: ASAP
- Travel Pay: Available
- Patient Population: Adults and individuals with chronic or complex care needs
Responsibilities:
- Provide skilled nursing care in home and group home settings
- Monitor patient health, administer medications, and perform routine nursing duties
- Document all care provided and communicate effectively with the care team
- Support patients with chronic conditions and disabilities
- Ensure compliance with care plans, physician orders, and regulatory requirements
Requirements:
- Active LPN License (Illinois)
- BLS/CPR Certification
- Home Health or Community-Based Nursing Experience Preferred
- COVID-19 Vaccination Card
- Valid Driver’s License and Reliable Transportation
- Strong Documentation and Communication Skills
