Overview
Join our dynamic healthcare team as a PRN/Part time/ or Full Time Occupational Therapist working onsite inside an Assistive Living Facility working with primarily Medicare Part B patient caseload, where your expertise will make a meaningful difference in patients' lives . This very flexible role offers the opportunity to work on an as-needed basis or consistent basis, and choose what days you work even when working part or full time. You will be providing essential occupational therapy services to a geriatric population. You will play a vital part in promoting independence, enhancing functional abilities, and supporting recovery through personalized care plans and innovative therapy techniques. Duties
- Assess patients’ functional abilities and develop tailored treatment plans to improve daily living activities (ADLs) and motor skills.
- Assist patients with activities such as dressing, grooming, feeding, and other essential self-care tasks to foster independence.
- Implement therapeutic interventions for individuals with disabilities, geriatrics, or those recovering from injury or illness.
- Collaborate with multidisciplinary teams to coordinate discharge planning and ensure seamless transitions between care settings.
- Conduct case management activities by monitoring progress, adjusting care plans, and documenting patient outcomes accurately.
- Provide education to patients, caregivers, and families about therapy goals, exercises, and adaptive strategies to enhance ongoing recovery.
Experience
- Assistive Living Facility, Skilled nursing facility, Home care experience or post-acute care background preferred, outpatient, with a focus on patient care continuity across settings.
- Knowledge of occupational health protocols and safety standards within clinical environments.

Pay: $48.00 - $55.00 per hour
