3-year-old female with history of Sandifer syndrome, severe GERO, quadriplegic spastic cerebral palsy, epilepsy, global developmental delay, failure to thrive, GJ tube dependence (switched from GT on 5/12/25), mechanical ventilation 24/7 via an Astral, tracheostomy, and laryngomalacia with intermittent strider.
Shifts:
Day, Monday - Friday 7am-5pm, Saturday or Sunday 8pm-6am,
Overnight, Monday - Friday 9pm-7am
Responsibilities
- Nurses without experience would be trained based on the patient needs
- Provide comprehensive nursing care to pediatric patients in a home health setting, ensuring safety and comfort.
- Administer medications and manage tube feeding while maintaining accurate electronic health records (EHR).
- Collaborate with families and healthcare teams to develop individualized care plans.
- Maintain CPR certification and demonstrate effective communication skills with patients and their families.
Requirements:
- Current Illinois RN or LPN license
- Minimum of one (1) year of nursing experience
- Private Duty experience preferred, but not required
- Current CPR certification
- Recent TB test (within last year)
- Yearly Physical document
- Excellent communication, organizational, and interpersonal skills required
- Skillful, accurate and timely clinical documentation and supply management
- Physically able to perform the job duties required for this position
