We are seeking a reliable and experienced RN who understands the importance of accurate documentation, time management, and follow-through in home health.
Position Summary
Heavenly Nurses Home Health is currently seeking an experienced and organized Registered Nurse (RN) to join our home health field team in Laredo, TX.
The RN will be responsible for completing comprehensive assessments, OASIS documentation, recertifications, resumption of care (ROC), start of care (SOC), and supervisory visits, in accordance with physician orders and Medicare guidelines.
The ideal candidate has strong home health experience, is confident in Kinnser/WellSky EMR, and understands that timely and accurate documentation is essential for compliance and reimbursement.
Responsibilities
- Perform Start of Care (SOC), Recertifications, Resumption of Care (ROC), Discharges, and supervisory visits.
- Complete comprehensive patient assessments and OASIS documentation accurately and thoroughly.
- Develop, implement, and update individualized plans of care.
- Ensure all documentation is completed and submitted in a timely manner, within required agency timeframes (24–48 hours of visit).
- Coordinate care with physicians and interdisciplinary team members.
- Communicate and obtain physician orders as needed.
- Supervise and guide LVNs and home health aides as appropriate.
- Provide patient and caregiver education regarding disease management, medications, and safety.
- Monitor patient progress and adjust care plans accordingly.
- Maintain strong time management skills to ensure completion of assigned visits and documentation.
- Follow through on all orders, documentation corrections, and compliance requirements.
Qualifications
- Current Texas RN license (Required)
- Minimum 1–2 years of home health experience (Required)
- Experience completing OASIS assessments (SOC, Recert, ROC, Discharge)
- Experience with Kinnser/WellSky EMR (Preferred)
- Strong knowledge of Medicare and home health documentation standards
- Excellent time management and organizational skills
- Ability to work independently in the field
- Strong clinical assessment skills
- Current CPR certification
- Valid driver’s license, reliable transportation, and current auto insurance
- Professional, dependable, and accountable work ethic
