Position Summary
Willow Park Health is seeking a compassionate and experienced Psychiatric Mental Health Nurse Practitioner (PMHNP-BC) to provide psychiatric evaluations and medication management services for older adults living in assisted living, independent living, and memory care communities across Long Island.
This is an excellent opportunity for a provider who enjoys building long-term patient relationships while maintaining an outstanding quality of life.
Responsibilities
- Perform comprehensive psychiatric evaluations
- Provide psychiatric medication management
- Develop individualized treatment plans
- Monitor treatment effectiveness and adjust medications as appropriate
- Collaborate with primary care providers, nursing staff, therapists, and families
- Document patient encounters accurately and efficiently using an electronic medical record
- Participate in interdisciplinary care planning
- Maintain compliance with Medicare and clinical documentation requirements
- Travel between senior living communities throughout Long Island
QualificationsRequired
- Active New York State Nurse Practitioner license
- Board Certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP-BC)
- Active DEA registration
- Medicare enrollment (or eligibility to enroll)
- Excellent communication and interpersonal skills
- Strong clinical judgment and documentation skills
- Reliable transportation and willingness to travel throughout Long Island
Preferred
- Two or more years of PMHNP experience
- Experience working with geriatric patients
- Experience in assisted living, skilled nursing, long-term care, or outpatient psychiatry
- Experience treating dementia, depression, anxiety, psychosis, and behavioral symptoms
- Experience using electronic medical records
Practice Setting
You will provide care within:
- Assisted Living Communities
- Independent Living Communities
- Memory Care Communities
There are:
- No hospital responsibilities
- No emergency department coverage
- No inpatient call
- No overnight responsibilities
