Are you passionate about helping youth and families thrive? Do you enjoy building relationships, coordinating services, and empowering youth and families through a strengths-based, Wraparound process? If so, Region 6 Behavioral Healthcare invites you to join our team as a Professional Partner.
Our Professional Partner Program provides voluntary care coordination for youth and young adults ages 3–26 who are experiencing mental health challenges and are at risk of out-of-home, school, or community placement.
As a Professional Partner, you will use the Wraparound process to partner with youth, families, and community providers to develop individualized plans that build on each family's unique strengths. You will facilitate face-to-face and team meetings, helping families identify supports, coordinate services, and connect with community resources to achieve positive outcomes.
What You'll Do
- Facilitate face-to-face and team meetings through the Wraparound process
- Develop and monitor individualized care plans
- Coordinate services and connect families with community resources
- Support families in achieving identified goals through collaborative, strengths-based planning
- Maintain accurate, timely documentation in the electronic health record (EHR)
- Manage program timelines and provide some evening availability based on family needs

Qualifications
- Bachelor’s degree in psychology, social work, human services, or a related field
- Knowledge of mental health, substance use disorders, and community resources
- Strong communication, collaboration, and relationship-building skills to effectively engage clients, families, and interdisciplinary teams
- Ability to work independently while managing a caseload of up to 15 clients with diverse needs
- Experience documenting services in an electronic health record (EHR) system
- Valid driver's license and current automobile insurance
