Job Summary
We are seeking a dedicated and compassionate Physical Therapist to join our dynamic healthcare team. Responsibilities
- Conduct comprehensive patient evaluations to determine physical therapy needs and establish clear treatment goals.
- Develop and implement individualized care plans utilizing evidence-based physiotherapy techniques tailored to each patient’s condition.
- Perform therapeutic interventions such as manual therapy, trigger point therapy, exercise programs, and modalities to facilitate recovery.
- Monitor patient progress regularly, adjusting treatment strategies to optimize outcomes and ensure safety.
- Collaborate closely with healthcare professionals to coordinate care plans effectively.
- Maintain accurate clinical documentation in accordance with medical terminology standards and electronic health records (EHR) management protocols.
- Provide discharge planning and patient education on exercises, mobility techniques, and home & community care strategies to support ongoing recovery.
Qualifications
- Valid licensure or certification as a licensed Physical Therapist in the relevant jurisdiction.
- Proven experience practicing in a physical therapy role within outpatient clinics, hospitals, inpatient units, or home health settings.
- Strong knowledge of anatomy, physiology, orthopedics, neurology, geriatrics, pediatrics, and related medical terminology.
- Experience developing treatment plans.
- Familiarity with clinical documentation standards and EMR (Electronic Medical Records) systems for accurate record keeping.
- Ability to work collaboratively with multidisciplinary healthcare teams to deliver comprehensive patient care.
- Excellent client communication skills with a focus on patient rehabilitation education and engagement.
- Working knowledge of discharge planning processes.
