The Care Coordination Resource Supervisor is responsible for assessing patients' clinical needs and ensuring accurate placement within Berkshire Health Systems. This role integrates care management skills with clinical expertise to make informed placement decisions, supporting both patient care and the organization’s strategic goals. The Care Coordination Resource Supervisor serves as the clinical expert and decision-maker for daily patient throughput and placement decisions, guiding the placement of patients to appropriate care settings.
The Care Coordination Resource Supervisor collaborates closely with the Patient Placement Assistant, and the Patient Placement Coordinator in admitting, to coordinate patient admission and transfer requests from the Emergency Department (ED) and Transfer Center across multiple facilities. This role also manages internal patient flow, facilitating movement between levels of care and bed assignments to ensure that patients receive care in the most appropriate setting.
By integrating clinical and financial components, the Care Coordination Resource Supervisor plays a critical role in making placement decisions that align with organizational objectives, ensuring both high-quality patient care and operational efficiency.
The Care Coordination Resource Supervisor supports staff development by using downtime in patient flow to mentor and coach team members on hospital workflows, policies, and procedures. This role reinforces consistency, strengthens compliance, and helps staff build confidence and efficiency in day-to-day operations while maintaining readiness to respond to patient care needs.
