Responsibilities
- Conduct thorough risk assessments to evaluate risk/needs.
- Develop individualized case plans
- Coordinate services and resources.
- Manage processes such as intake, assessments, discharge planning to optimize resource use.
- Collaborate with stakeholders, and community partners
- Maintain strict compliance while handling sensitive records and information.
Skills
- Proven experience in corrections environments, case management, and data entry.
- Strong knowledge of criminal justice systems, and/or correctional settings
- Ability to perform comprehensive assessments with attention to detail
- Excellent communication skills.
- Background in social work or mental health counseling is highly desirable for supporting behavioral health needs.
