RN - Home Health - Local or Travel, Northridge, CA

Mon - Fri, 8am - 5pm, 40 hours per week
Coverage area: San Fernando Valley and Santa Clarita May be asked to help in other territories within facility's service area as needed which is about a 60 mile radius from the Facility address
RN productivity expectation for 40 hrs = 28 - 32 points/wk SOC 2.5, ROC 2.0, Discharge (OASIS) 1.5, Recertification- 1.5, Routine visits-
1 Must have recent HH exp 2024-25
HomeCare HomeBase experience required
Any overtime must be pre-approved and requires written approval from Administrator 2 weeks cancellation
Pay: $2,500.00 - $2,700.00 per week
